Student Acknowledgement

The student understand that by not completing the requirements set before them by the deadline(s) determined by the instructor, the student will receive a zero for the assignments and the grade earned in the course.

Copies of this completed form will be distributed to:

  1. Instructor
  2. Student (Instructor will share)
  3. Program Director
  4. School of Social Work Director
  5. School of Social Work Admin III

Upon completion of the Incomplete ā€˜Iā€™ and for official submission of a Change of Grade utilized the Change of Grade link through the Office of the University Registrar.
