The student understand that by not completing the requirements set before them by the deadline(s) determined by the instructor, the student will receive a zero for the assignments and the grade earned in the course.
Copies of this completed form will be distributed to:
- Instructor
- Student (Instructor will share)
- Program Director
- School of Social Work Director
- School of Social Work Admin III
Upon completion of the Incomplete āIā and for official submission of a Change of Grade utilized the Change of Grade link through the Office of the University Registrar.
